The data provided through the pre-booking form is used solely to manage your contact or appointment request: to reply to you, confirm availability and organise the practice diary. If you tick the optional communications box, also to send you educational health information.

The legal basis is your express consent (art. 6.1.a GDPR) when submitting the form, including explicit consent for the reason-for-consultation field (art. 9.2.a GDPR, health-related data). You may withdraw it at any time.

Data is not shared with third parties except where legally required. Data is hosted with service providers (hosting and database) under GDPR-compliant data processing agreements.

Request data is kept for as long as necessary to manage it and, at most, for the period defined in the practice's internal policy, after which it is deleted or anonymised.

You may exercise your rights of access, rectification, erasure, objection, restriction and portability by writing to the controller's email (to be confirmed). You may also lodge a complaint with the Spanish Data Protection Agency (aepd.es).

Full name, phone, email, contact and time preferences, type of visit, modality (private/insurance) and general reason for consultation. For anti-spam security purposes a pseudonymised identifier (hash) of the connection is stored.

Data is transmitted encrypted (HTTPS) and stored in a database with strict access control. Access to requests is restricted to the practice's authorised staff.

Do not send medical reports, full diagnoses or detailed clinical information through the form. This channel is not part of your medical record and does not replace a medical consultation.
